Service Provider QIP Survey & Compliance Status

Attention Service Providers:

You may now review your Quality Incentive Program (QIP) survey and compliance status as a provider subject to rate reform who served individuals during fiscal year (FY) 2024–2025.

As a reminder, to earn your QIP rate for FY 2026-2027,a providers must:

1. Be validated in the provider directory
2. Maintain compliance with Electronic Visit Verification (EVV) guidelines
3. Maintain compliance with independent audits/reviews
4. Maintain compliance with HCBS Final Rule regulations
5. Complete the appropriate QIP surveys

To check the status of a provider in any of these areas, please visit DDS website by clicking this link visit the QIP webpage.  

Provider statuses can be found on the “Provider Survey Status” tab.

Scroll down to access each of the status trackers. Trackers will be updated each Monday through February 28, 2026. In addition to the status trackers, the QIP webpage also contains training material, videos, user guides, and more resources for providers and regional centers to better understand the QIP and individual surveys.
