Quality Incentive Program: Provider Eligibility to Earn Quality Incentive Rate for Fiscal Year 2026-27
On October 9, 2025 DDS released a directive to service providers about Provider Eligibility
for the Quality Incentive Rate (100% benchmark rate) for Fiscal Year 2026-27.
On July 18, 2025 DDS released a directive to existing service providers that existing that
they must be in compliance with all the requirements, as applicable to each provider, for
Electronic Visit Verification (EVV); Home and Community-Based Services (HCBS) Final
Rule, and Independent Audit and Reviews by February 27, 2026, to be eligible to earn the
quality incentive rate component(s) for FY 2026-27.
In order to be eligible for all or a portion of the Rate Model Quality Incentive Program (QIP)
effective July 1, 2026, eligible service providers will need to be enrolled in the Service
Provider Directory and complete and submit the DDS Survey(s) required for their service
category. Beginning November 3, 2025, providers that are eligible to participate in the
survey received an e-mail from DDS with a “HYPER LINK” to complete the survey. A
separate e-mail was sent to provider that needed to complete more than one (1) survey.
The e-mails were sent to the Administrative e-mail listed in the Service Provider Directory

All surveys need to be completed and submitted no later than January 31, 2026
for you to be eligible for all or a portion of Rate Model QIP.

For Prevention and Wellness survey and Employment survey, using the email address
listed in the Service Provider Directory, please search your inbox, spam folder, and junk
folders for an email from Department of Developmental Services Office of Quality
Assurance (DDS OQA) or DDS-OQA@qualtrics-survey.com with the subject line [NEW
LINK].
For Provider Capacity survey, using the email address listed in the Provider Directory,
please search your inbox, spam folder, and junk folders for an email from
noreply@gemailserver.com.
If you meet the eligibility requirements and are not able to locate the survey link,
please send an e-mail to QIPquestions@DDS.CA.GOV with your vendor name, vendor
ID/Number, and eligible service codes in order for DDS to provide further assistance.
If you received an link to complete a survey for a vendor number that is not assigned
to you please send an e-mail to QIPquestions@DDS.CA.GOV with your vendor name,
vendor ID/Number, incorrect vendor number(s)and service code(s) in order for DDS
to DDS further assistance.
Please note that DDS can only communicate via email with email addresses registered in
the Provider Directory. If you need to update your information in the Provider Directory,
please reference this user guide.
