Intake and assessment services will review the request for service based on the following eligibility criteria:

• any person believed to have a developmental disability
• any child under 3 who is at risk of becoming developmentally disabled
• or any person who has a high risk of parenting a child with a developmental disability.

An assessment team will complete the needed evaluations. If an applicant is determined to be ineligible, this decision may be appealed by requesting a Fair Hearing. (See Appeal Process).
